Chirpy Hearts Assisted Independent Living offers NDIS-funded accommodation services in Melbourne for participants with psychosocial and other disabilities, providing individualised support to help them achieve their living goals and needs.

Tailored Support in Melbourne

Participants receive customised assistance to find suitable accommodations in South, East, and Central Melbourne.

Flexible Living Arrangements

Participants can choose between shared or individual accommodations, with shared common areas and furnished residences.

Clean and tidy bedroom with natural light - professional NDIS home maintenance and cleaning services. Accommodation & Supported Independent Living Services

Qualified Staff and Safe Environment

Skilled staff ensure participants feel safe and supported while enhancing their independent living skills.

Comprehensive Daily Support

Daily activity support from workers includes cooking, cleaning, appointment management, medication support, exercise, and social activities.

Direct Support provided by a registered NDIS Provider

Support Coordination

At Chirpy Hearts we have qualified staff to have an understanding of your Support Coordination needs. Support Coordination is a capacity-building support which helps you to:
• Understand and use your NDIS plan to achieve your goals
• Connect you with NDIS service providers, community, mainstream and other government services
• Monitor service delivery, reporting requirements, fees and service agreements.

Direct Support & Independent Living Skills

At Chirpy Hearts our Direct Support Workers create a safe environment to help with ADL and your other needs. We are creative in bringing you services that are tailored only for you.  We understand that your individual needs are unique. We work closely with you and your loved ones to develop a support plan that meets your specific needs and goals, with focus on promoting independence, choice and control.

Accommodation

At Chirpy Hearts we create easy access to a variety of Living Options. We can assist you in implementing your NDIS plan, relating to any of your Individual Living and Housing goals. We make sure your individual circumstances are assessed in a way that you feel supported and listened.

Cleaning & Maintentance

This is a service we provide with our professional cleaners and handyman. We can help you with daily assistance, maintaining the hygiene of your home, and creating a safe and secure environment by repairing and maintaining your home and yard. Our cleaners and handyman would give you quotes based on the size of your project.

NDIS participants and support workers enjoy a communal meal outdoors, promoting community engagement and social inclusion.

Group Programs

Come along to day trips visiting Victoria's landmark beaches and parks, have lunches, chat with peers and enjoy socialising. We are having a great time, building new friendships, where people long to see each other at the next event! Happy to build males only groups based on demand.

What is NDIS?

The National Disability Insurance Scheme (NDIS) is a program that provides support and services to people with disabilities in Australia. It aims to assist individuals in achieving their goals, enhancing their independence, and improving their overall quality of life.

Who is eligible?

To be eligible for the NDIS, you must be under the age of 65 and have a permanent and significant disability that affects your ability to participate in everyday activities. You must also be an Australian citizen or hold a permanent visa.

How to apply?

To apply for the NDIS, you need to complete an Access Request Form and provide supporting documentation. This form can be obtained from the National Disability Insurance Agency (NDIA) website or by contacting their helpline. Once your application is submitted, it will be assessed by the NDIA.
